Patient Information

  • Please arrive at least 25 minutes early to your first appointment in order to complete our electronic paperwork. Bring any additional completed forms to your first appointment, along with your medical records, referral (if your insurance requires one), valid medical card, and drivers license or another form of photo identification. The privacy notice is provided here for your reference only.

  • We charge $50 for all returned checks. Twenty-four hours’ notice is required for cancellations. A $50 “No Show” fee will be charged for any missed appointments not canceled with more than 24 hours’ notice. The practice reserves the right to increase this fee for habitual no-shows.

    If your insurance is an HMO, you MUST have a referral for EVERY visit. Your Primary care doctor writes the referral and you must present it each time you are seen in our office. Occasionally, your PCP will allow for more than 1 visit. If this is the case, the first must occur within 30 days of the date of the referral or it will be considered invalid. Additionally, depending on your plan, your referral may expire before all of the visits are used. In this case, you will be required to obtain a new referral prior to your office visit. Please note that it is the patient’s responsibility to obtain a referral and verify that it is accurate and current.

    Our office has a 48 business hour refill request turn-around time. Please make sure that you request your CONTROLLED refill prescriptions by calling our office 2 to 3 business days before your medication will run out. In addition, if you are in need of a NON-CONTROLLED medication refill, you must contact your pharmacy directly and have them forward the refill request to our office, again you will need to make this request 2 to 3 business days prior to your medication running out.

    There is a $20 fee for not paying your co-pay at the time of service.

    If you are 10 minutes or more late for your appointment, we reserve the right to reschedule your appointment.

    If you are unable to pay for a bill in a timely manner, please contact our Financial Care Coordinator at 410-535-2500 extension 304 to set up payment arrangements.

  • Do's and Don’ts for Patient QEEG Preparation

    Patients should have clean, dry hair. No hair products including hairspray, gels or oils should be applied. Patients should avoid any hairstyles that consist of wigs, hair extensions or braids.

    Patients should be aware that a sticky paste/gel is used during the EEG that will leave the hair stiff and sticky after the test has been preformed. It is suggested that patients bring a hat, comb or have a plan to wash their hair after the testing.

    Patients should avoid any kind of caffeine at least six (6) hours prior to the EEG test; this includes coffee, tea, and soda.

    Patients should have a good night's sleep the night before the EEG unless otherwise directed by your physician. For patients having a sleep deprived EEG they should sleep no more than four (4) hours the night before the test.

    Patients should/should not take any medications as directed by their physician. Patients with ADD/ADHD should avoid taking any medication prior to their test if they can handle it. If there are any questions regarding medications, patients should contact their physician directly to inquire.

    Patients should plan for their EEG test to take approximately an hour to an hour and a half.

    Patients family members may be asked to remain in the waiting room or outside of the EEG laboratory during testing in order for the patient to remain undisturbed.

Our office hours are from 7:30am to 4:30pm Monday through Friday. 

New Patients who prefer to fill out their registration packet in the office should arrive 25 minutes early. For your convenience, you can click the link on the patient forms tab, print, and bring the completed registration packet with you. If you chose this option, please arrive 15 minutes early. Please make sure to bring the following to every appointment:

  • Valid Driver's License or Photo Identification

  • Valid Insurance Card

  • Referral from your Primary Care Physician (if needed)

  • Order from Referring Physician

  • Results of Diagnostics Testing (MRI's, CAT Scans, etc.)

  • List of Prescriptions and Medical conditions

  • Medical Records
