New Patient Information

Our office hours are from 7:30am to 4:30pm Monday through Friday. 

New Patients who prefer to fill out their registration packet in the office should arrive 25 minutes early. For your convenience, you can click the link on the patient forms tab, print, and bring the completed registration packet with you. If you chose this option, please arrive 15 minutes early. Please make sure to bring the following to every appointment:

  • Valid Driver's License or Photo Identification

  • Valid Insurance Card

  • Referral from your Primary Care Physician (if needed)

  • Order from Referring Physician

  • Results of Diagnostics Testing (MRI's, CAT Scans, etc.)

  • List of Prescriptions and Medical conditions

  • Medical Records

Sarah Zinchiak, CRNP, is not just your average Certified Registered Nurse Practitioner. With her unwavering dedication and genuine care for patients of all ages, Sarah has earned a stellar reputation as a trusted healthcare professional. She is a graduate of the esteemed West Virginia University, where her passion for improving the well-being of individuals took root. Sarah brings a wealth of knowledge and expertise to the practice, making her the ideal provider for your medical needs.

Tammera Schmalz, PhD

Dr. Tammera Schmalz, PhD is a licensed clinical psychologist that has continuously provided warm, dependable, and unwavering psychological care to her clients for over 30 years in Calvert County, Maryland.

David Ims, LCPC, BCN, Q-EEGD

P. David Ims, LCPC is a Licensed Mental Health Counselor in Maryland with a passion for the intersection of mental health and applied neuroscience. He spent 10 years as an EEG technologist in hospital and then outpatient settings, where he developed a skillset in electroencephalography and neuro-therapeutic techniques.

Alex Kuo, LCSW-C

Alexandria Kuo is an LCSW-C, or a Licensed Certified Social Worker – Clinical. She received her BSW at Salisbury University, her MSW at Catholic University of America in 2016, and has had her independent license since 2018. Alex has experience working with adults, adolescents, and children, though is specifically trained with adolescent girls ages eight to eighteen.

Amye Scrivener, LCSW-C

Amye Scrivener, LCSW–C is a social worker in Southern Maryland. She has an expansive knowledge of the disciplines of neurofeedback therapy, talk-based therapies such as CBT and DBT, and the clinical application of various modalities of neuromodulation/PEMF.

Danielle Anderson, LCPC

Danielle Anderson, LCPC is a Licensed Clinical Professional Counselor. She received her M.S. in Counseling Psychology from Loyola University Maryland in 2013, and her B.A. in Psychology from James Madison University in 2011. As part of the staff at Chesapeake Neurology Associates, Ms. Anderson provides outpatient therapy to adults, families, adolescents, and children using various treatment modalities.
